Kids Love Quality Time

January 6th, 2010 Comments off

We love to spend time with our children and bond with them everyday. But as career-oriented and working moms, we rarely have the chance to do so because of our hectic schedules. We long to be with them for every minute of their lives, but we simply can’t. We just can’t find much available time to do so.

Because of our hectic schedule, we are already tired and exhausted whenever we go home. And one of the quickest and simplest form of bonding we could give our children is watching television together. Just by doing so, we think that we have already spent time with them. Watching tv is our way of bonding with them, at the same time, we get to relax our tired body.

Watching tv can be fun, but letting your children watch too often for you to spend time with them is not quality time. While watching tv can be educational especially if they are watching informative programs, too much of it is not a healthy bonding for you and your children.

TV can give useful information, but toddlers learn best by actual touching, tasting, smelling, hearing, and seeing. We say it is educational, but if we are perfectly honest with ourselves, the real reason we allow our children boob tube time is that it is convenient. The colorful, flashing images grab their attention and glue them to their seats. We get to cuddle them without worrying about keeping them busy. TV gives us the luxury of low-maintenance quality time.

But most of the time, despite the convenience, we still feel bad about boob tube bonding. So we often rationalize- we think of the hundred and one things that our precocious children are learning from TV. Unfortunately, there is increasing evidence of the harmful effects of TV on little children.

TV does not allow experimentation- they passively watch the images and absorb pre-processed facts. They can’t interact with it, and because there is no interaction, there is no feedback. A habit of passive learning dampens curiosity and weakens problem-solving skills. Children grow used to being given information, not actively processing experiences for hidden facts and concepts.

The bottom line is, you don’t need energy to watch TV, but you do need to invest time in support activities to glean any educational value from what your toddler watches. A 30-minute Barney tape on numbers has to be accompanied by games and activities where you and your children learn to count together.

Having quality time with your kids does not rely solely on watching TV. There are a lot of fun and educational activities that you could do together to strengthen your bond. Playing is one important bonding activity that you could share with your children.

There are a lot of educational toys available in stores and even online that are great for your children. For your toddlers, Ride-on toys and pedal cars are a sure hit to them. Toddlers love toys that are mobiles. You can bond with them by teaching them how to play and ride on those toys. Its double the fun and enjoyment, plus educational benefits that could help your children in their learning stage.

Being tired and exhausted is not a reason to disregard your time with your children. Spending quality time with them is the most important aspect of a good parenting.

What Are the Common Medications Used for Anxiety Disorder

November 25th, 2009 Comments off

Generally, most psychiatrists will prescribe benzodiazepines for anxiety, with the most popular being Ativan (lorazepam) and Xanax (alprazolam). While these medications are usually the first choice, there are however different options used that were not originally created to treat anxiety.

Certain antidepressants are also used in the treatment of anxiety. These medications are different from anti-anxiety medications in several different ways such as a low risk of dependency and abuse. In addition, antidepressants can take anywhere from four to six weeks to fully take effect. These medications can not be used for sudden symptoms of anxiety either.

SSRIs are the most commonly prescribed antidepressants to treat anxiety. SSRIs or selective serotonin reuptake inhibitors include Prozac, Zoloft, Paxil, Celexa, and Lexapro. These medications are used to treat both generalized and severe anxiety such as OCD or obsessive compulsive disorder. As with all prescription medications, there are some risks of side effects. The most common side effects include weight gain, stomach upset and nausea, sexual problems, sleepiness and dizziness, nervousness, and headaches. In addition, there are reported cases of more severe side effects which include suicidal thoughts, hostility, and aggression. An additional risk involved with taking antidepressants and anti-anxiety medications is their ability to actually increase symptoms in both disorders.

SSRIs:

These medications are used to treat anxiety by correcting the chemical imbalance in the brain involving serotonin. SSRIs accomplish this by making more of the neurotransmitter available to the receptor sites of the post-synaptic nerve.

Buspirone (BuSpar) is a newer anti-anxiety drug that acts like an SSRI by increasing serotonin in the brain (subsequently reducing brain activity). BuSpar also reduces dopamine, a neurotransmitter (also a medicated substance) that increases heart rate and blood pressure. It starts working in half the time of antidepressant drugs, and lacks many of the side effects of anti-anxiety drugs, such as sedation. BuSpar also doesnt cause loss of memory or coordination and is not habit-forming, two problems with drugs such as Ativan. BuSpars effectiveness is limited to generalized anxiety disorder.

Since prolonged anxiety can lead to high blood pressure and heart problems, beta blockers are sometimes prescribed to treat these problems. They also help deal with physical symptoms of anxiety, such as dizziness, sweating, trembling voice, and rapid heart rate. Beta blockers are best for phobias. Side effects of beta blocker use include nausea, sleepiness, lightheadedness, and a very slow pulse.

Your doctor will be able to determine whether medications such as the ones listed above are necessary. He or she will ask you a series of questions as well as perform some physical tests to decide which medication will befit you the most. It is very important that these drugs take time to work at their optimum capacity and there may be a series of medication changes dependent upon any side effects endured.

Dementia Managing

October 31st, 2009 Comments off

Part of the problem in finding drugs which may be effective for dementia is that our ideas about what constitutes dementia have been undergoing radical change in recent years. It had been traditional to distinguish between Alzheimer’s dementia, or senile dementia of the Alzheimer’s type (SDAT) and multi-infarct dementia (MID), which is theoretically caused by small strokes which insidiously pick off brain tissue to the point where an individual’s cognitive function is compromised.

It was originally thought that MID accounted for 60%+ of the dementias. Accordingly, early attempts to treat the dementias concentrated on the multi-infarct dementias. The initial hypothesis was that these multiple small strokes were being caused by a process of hardening of the arteries, sometimes called arteriosclerosis and sometimes atherosclerosis (although these terms refer to two quite different disorders) which impaired blood supply to the brain. The logical treatment, therefore, for this condition was to attempt to dilate blood vessels. This led to the use of a wide number of vasodilating drugs such as hydralazine.

It is quite rare now for such drugs to be used for this purpose. Arguably, if anything, such treatment may have made the condition somewhat worse in that a potential effect of vasodilators is the reduction of blood pressure and reducing blood pressure would mean that the brain would be less perfused with blood, as one of the functions of blood pressure in the first instance is to provide the propulsive force to send blood up against the force of gravity to perfuse the brain.

Stage 2

More recent attempts to treat the dementias have proceeded on the basis that Alzheimer’s dementia is the commonest form of dementia. For many years, the term Alzheimer’s dementia was reserved for dementias that came on before the age of 65 (for this reason it was also called persenile dementia), which were not obviously caused by strokes. It was conceded that there was another dementia that was like Alzheimer’s dementia, which appeared to come on after the age of 65 but this was thought to be less common. Distinctions on the basis of age have now collapsed and both dementias of the Alzheimer type are now called senile dementia of the Alzheimer type. The amalgamation of these two groups led to an awareness that Alzheimer’s-type dementia is the commonest form. The primary therapeutic focus in the field, therefore, has been on an attempt to reverse the deficits which are supposed to be present in SDAT.

In particular, it has been held that in Alzheimer’s, there is a dysfunction of cholinergic pathways in the brain, for which there are both historical and clinical reason. Historically, when early work in psychopharmacology began, there were only four known neurotransmitters – noradrenaline, 5-HT, dopamine and acetylcholine (ACh). Noradrenaline quickly became the neurotransmitter involved in depression and mood disorders. Dopamine was known to be involved in Parkinson’s disease, and, when it became clear that neuroleptics acted on it, schizophrenia, after which the psychoses in general came to be seen as disorders of dopamine neurotransmission. For the most part, 5-HT was associated with either depression or anxiety. This left ACh without a function. It seemed convenient to parcel it out to the dementias.

There was, in addition, some clinical evidence in favour of an association between the cholinergic system and dementia. Part of the reason for this claim can be seen in a number of the chapters of this blog, in which drugs with anticholinergic effects have been noted as potentially causing amnesia or confusion (see The Management of Side Effects & Side Effects of Antidepressants articles).

Stage 3

In the last 5 years, a number of other dementias have been described. A distinction has been drawn between cortical and subcortical dementias. The cortex of the brain is the area responsible for higher cognitive functions, such as speaking, reading, planning and executing actions, etc In the cortical dementias, memory is usually the function most noticeably affected but those who are affected also have problems with planning even simple functions such as dressing and they typically cannot read, draw or execute any complex tasks. Alzheimer’s and MID are cortical dementias. There are also subcortical parts to the brain which are common to humans and other mammals. They involve a number of what are termed midbrain and brainstem structures.
